is now available to download in PDF and Word formats. Simply print out form, send it to Evergreen Valley with check and membership card will be mailed to you.
Remember that volunteering for 5+ club functions will get you a Free 08/09 Club Membership!

For your convenience, you can also Join the Club over the phone with credit card at Milton Pump Shop at 603-652-7100. Join the club and register your sled at Rochester Sports in Rochester or Milton Pump Shop.
